Enrollment Procedure
Step I
(a) New Student (Freshman): Report to the Admission Clerk any day during the enrollment period. Submit the following documents:
- (1) Form 138 2 copies
- (2) Birth Certificate 1 photocopy
- (3) Certificate of Good Moral Character 1 copy
Fill up the Personal Information Sheet and get your student number.
(b) Transferee: Report to the college dean any day during the enrolment period. Submit a photocopy of your grades in subjects you have taken for evaluation and advisement.
Also, submit the following documents:
- (1) Certification of grades Original
- (2) Certificate of honorable dismissal Original
- (3) Birth Certificate 1 photocopy
- (4) Certificate of good moral character Original
Then, proceed to the Admission office where you will fill up the Personal Information Sheet and get your student number.
For those enrolling in a second course and who finished the first course in another school, they should submit
- (1) Transcript of records Original
- (2) Certificate of honorable dismissal Original
- (3) Birth certificate 1 photocopy
- (4) Certificate of good moral character Original
(c) Re-enrollee: Report to the Admission Clerk any day during the enrollment period. Fill up a re-enrollee form. Get your student number.
(d) Re-enrollee and Shifting: Report to the Admission Clerk. Fill up re-enrollee and shifting forms. Get your student number. No student shall be allowed to shift after he/she has officially enrolled in a certain course/program.
(e) Cross-enrollee: Report to the Admission Clerk any day during the enrollment period. Submit your cross-enrollment form/permit to enroll form. Fill up the personal information sheet. Get your student number.
(f) Old Student: Register on the day scheduled for enrollment of your year level. See your schedule for registration. There is no need to report to the Admission Clerk.
(g) Old Student and Shifting: Register on the day scheduled for enrollment of your year level. Report to the Admission Clerk. Fill up shifting form.
Step II
Pay your registration fee at the teller’s section. Get your pre-enrollment form.
Step III
(a) New Student (Freshman): Proceed to the Dean’s Office for advisement. Accomplish your pre-enrollment form then submit it to your Dean or his/her duly designated registration assistant for approval of your enrollment.
(b) Re-enrollees and Cross-enrollees: Proceed to the Dean’s Office. Present a copy of the subjects you have taken and grades already earned for evaluation. Accomplish and submit your pre-enrollment form to your Dean or his/her duly designated registration assistants for approval of your enrollment.
(c) Transferees: Proceed to the dean's office. Accomplish and submit your pre-enrolment form to your dean or his/her duly designated registration assistants for approval of your enrolment.
(d) Old Students: Secure your Computerized Evaluation List (CEL) from the Dean’s Office. The checklist carries the subjects you were enrolled in during the preceding school term, your grades in the subjects, and a list of subjects which you may take for the current school term. Choose your subjects from the list, plot your schedule, then accomplish and submit your pre-enrollment form to your Dean or his/her duly designated registration assistants for approval of your enrollment. Be sure to plot the total number of units you intend to enroll in during the school term before seeking the approval of your Dean to avoid adding of subjects/units later.
Students who desire to take ROTC: Proceed to the Department of Naval Science and Tactics for registration in NROTC before securing the approval of your enrollment from your Dean or his/her duly designated registration assistants.
Step IV
(a) New Student (Freshman): Leave your accomplished pre-enrolment form in the dean's office. The office will take responsibility for the encoding of your subjects and schedule. Go back to the dean's office after at least an hour to get your batch number.
(b) Old Student, Transferees, Re-enrolees and Cross-enrolees: See the encoding clerk of your college for the encoding of your subjects and schedule. In the event that any of the sections/subjects you have chosen is closed, you will be asked by the computer encoder to select a new section/subject from among the open codes. Go back to the Dean for his/her approval of the new code/subject.
Step V
(For new enrolees or for enrolees with expired IDs) Proceed to the Photo & Supplies Section for ID picture-taking.
Step VI
Secure your Certificate of Matriculation (COM) from the Student Accounts Section (Window 11) or from the Second Floor Library Building, whichever is designated as releasing center for your college. Present your batch number and your ID slip to the COM releasing personnel.The COM will be released only after a student has had her ID picture taking.
